DPW Facing Thousands in Fines for Fatal Accident

The city of Indianapolis is facing thousands of dollars in fines in connection to an accident that killed a trash collector.

DPW Public Information Officer Lesley Gordon says the Department of Labor found ten violations while investigating the death of 45-year old Thomas Welch. Welch died after being pinned between a tree and DPW truck on April 8th. Gordon says the violations include 5 serious violations, four knowing violations and one non-serious violation.

Two DPW employees were fired and two others were disciplined for backdating maintenance logs and altering training documents after the incident. Gordon says the discipline was not related to the IOSHA investigation.

Gordon says there is no timeline for when the fines will be resolved.
